What shared support “Home to Home” is all about …
Parents/ guardians / families / whanau are important people and
sometimes need to have a break from the 24 hour care responsibility
that a person with a disability requires.
The family unit needs to maintain a healthy balance and a quality
of life.
The HOME to HOME service has been designed to meet parents / guardians
/ family / whanau needs in providing relief care (‘time out’)
for a person with a disability in their own home or the carer’s
home.
Shared support also provides the opportunity for a person with a
disability to make new friendships outside the family home.
Shared Support Home to Home is:
A service where all involved believe that they can work closely
together to provide the best possible care.
A partnership between parents / guardians / families / whanau and
carer, i.e. carers are people who provide relief care.
Our Service Provides…
Thorough screening of carers (and other adults in the carer’s
home) prior to registration with Home to Home. This is achieved
through:
- A series of interviews
- A police check
- A doctor’s check
- A DSW check
- 2 references
Training and support for carers
A matching process is carried out prior to any placement.
This is achieved through the involvement and interaction of parents
/ guardians / families / whanau / carers and the Co-ordination of
the Home to Home service.
Monitoring and evaluation of placements on a regular basis to ensure
the needs of all parties are being addressed.
Standards developed to ensure that the best possible care is able
to be provided.
Home to Home is a 24 hour back-up on call service for families that
need short / long term care at any time.
Funding…
The carers are completely funded by the Health Funding Authority
“Carer Support Scheme”.
Eligibility to obtain relief care (‘time out’) and carer
support is through a Needs Assessment. This assessment will
determine the number of carer support days available to meet the
needs of parents / guardians / family / whanau.
There is no charge involved to either party when registering with
CCS for this service.

Carer support brings relief to parents / guardians / families /
whanau.
This service provides a home environment for people who need relief
care (‘time out’).
Ensures that carers (and other adults in the home) who provide their
service to the parents / guardians / family / whanau are assessed
as being suitable and compatible to the families and the person
with a disability.
Close monitoring and evaluation is provided by the Carer Support
Co-ordinator to ensure that the needs of all parties are addressed.
Carers attend training, which includes First Aid and Disability
Awareness to ensure that the best possible services are given.
